The address was matched to the click point using the Pick Address from Map tool in the Interactive Rematch dialog box with ArcMap. PA = Point was places by Pick by Address. The address was matched to closest address with the Pick Address from Map tool.
Side	ESRI item which denotes the side of the street to which the address was matched (for geocoding service styles that can match an address to a particular side of a street). Records marked with an "*" have not been GeoCoded, these records have another type of spatial reference source.
Match_addr	ESRI item which denotes the full street layer address to which the address was matched.
FAC_ID	The facilities identification number.
NAME	The name of the facility.
DISTRICT	The FDOT district that the facility falls within.
STATUS	The status of the facility, i.e. the facilities Quality Control Plan.
ADDRESS	The address of the facility.
CITY	The city that the facility is located in.
STATE	The state of the facility
ZIPCODE	The zipcode of the facility.
CONTACT	The contact person for the facility.
PHONE	The phone number of the facility.
EMAIL	The email address for the facility.
MAILING	The mailing address for the facility.
M_CITY	The city that the mailing address is located in.
M_STATE	The state that the mailing address is located in.

DATA LINEAGE SUMMARY:

GeoPlan downloaded the source for this dataset from the Florida Department of Transportation, Materials Office website on October 7th, 2008 (http://www.dot.state.fl.us/statematerialsoffice/quality/programs/qualitycontrol/materialslistings/postjuly2002.shtm). The data was received as an xls and then converted to a dbf. The dataset was then geocoded based on the provided addresses. Non geocodeable addresses and sites outside of the state of Florida were deleted. There were 6 records that fell into these categories. -The addresses were geocoded against ARC Logistics Route GDT Roads Shapefile created using FGDL Albers HPGN map projection - All fields were upcased. - A DESCRIPT field was added based on NAME - A FGDLAQDATE field was added based on date downloaded from source - File was originally named producers.xls Process Date: 20081007 Data imported to ArcSDE and exported as a shapefile. Process Date: 20081021
