GENERAL DESCRIPTION:
This dataset contains locations of facilities in Florida that have lasers on site. This dataset is based on information received from the Florida Department of Health, current up to March 2008. This dataset contains fields denoting the physical address and the specific laser information for that site.

FEATURE ATTRIBUTE TABLES CODES AND VALUES:

Item Description
OBJECTID Internal feature number.

STATUS ESRI item which denotes the status of the geocoded address whether it was matched or not

SCORE ESRI item which denotes the score with which the address was matched to a feature in the reference data

SIDE ESRI item which denotes the side of the street to which the address was matched (for geocoding service styles that can match an address to a particular side of a street).

MATCH_ADDR Address used to geocode data. This address will not match the provided ADDRESS field if it did not originally geocode. 597 addresses were corrected that did not originally geocode.

OWNER_REG The owner's registration number

DATE_ADDED The date the laser was registered

NAME Laser facility Designation

ADDRESS A description of a facility's physical location providing direction for delivery and provision of emergency services

CITY City of facility's physical location

ZIPCODE US postal delivery designation of facility's physical location

PHONE The phone number of the facility

COUNTY The county where the facility is located

QUANTITY The quantity of lasers at the facility. The quantity field should always have a positive integer in it, or a zero. If you see zero, it means that laser was removed from registration at some point after initially being registered.

MEDIUM The medium of the laser at the facility

CLASS The FDA's laser device classification. These should be either Class 1, 2, 2A, 3A, 3B, or 4. 4 is the most powerful, 1 the least.

DURATION Duration describes pulse attributes of the laser.
P = Pulsed Laser

C = Continuous Laser

B = Both pulsed and continuous


PWR_ENERGY Used with PWR_UNITS to describe the maximum energy in joules or millijoules, or maximum power in watts or milliwats

PWR_UNITS Used with PWR_ENERGY to describe the maximum energy in joules or millijoules, or maximum power in watts or milliwats

WAVELENGTH Wavelength in nanometers

MAKE The make of the laser at the facility

MOBILITY The mobility of the laser, either Fixed or Mobile

LOCATION Additional Location information for the facility

DESCRIPT FGDL added item based on NAME

SHAPE Feature geometry.

FLAG Type of update that occurred
A = Attribute edit

S = Spatial Edit

B = Both attribute and spatial edit

N = New

D = Delete

NIN = Not in new


UPDATE_DAY The last day the file was updated

LAT_DD Latitude in Decimal Degrees

LONG_DD Longitude in Decimal Degrees

USNG_FL_1K Facility's 1-kilometer United States National Grid (USNG) address. The USNG is an alpha-numeric reference system based on the UTM coordinate system and is similar to the Military Grid Reference System. Use of the USNG ensures a uniform grid mapping and positional reporting system for search and rescue, emergency planning, response, and recovery. A note about data scale: 

Scale is an important factor in data usage.  Certain scale datasets
are not suitable for some project, analysis, or modeling purposes.
Please be sure you are using the best available data. 

1:24000 scale datasets are recommended for projects that are at the
county level.
1:24000 data should NOT be used for high accuracy base mapping such
as property parcel boundaries.
1:100000 scale datasets are recommended for projects that are at the
multi-county or regional level.
1:125000 scale datasets are recommended for projects that are at the
regional or state level or larger.

Vector datasets with no defined scale or accuracy should be
considered suspect. Make sure you are familiar with your data

DATA LINEAGE SUMMARY:
Laser oriented facility locations were obtained from the 
Florida Department of Health. GeoPlan received this data 
in Excel format in two separate spreadsheets and converted 
them to dbf.  One spreadsheet contained facilities and addresses 
and one contained individual laser information. These two tables 
were joined through the owner registration number. The facility 
to laser relationship is one to many therefore duplicate locations 
do exist. These duplicates represent unique lasers at the same 
location or facility. The dbf was geocoded against ARC Logistics 
Route GDT Roads and converted into a shapefile with the FGDL 
Albers HPGN map projection. Records with no address or bad
addresses were omitted. 1043 records were removed that had a 
0 in the QUANTITY field. Zeros show that the laser was removed 
from registration at some point after initially being registered.
